St. Joseph Jr-Sr High School    Hilo, Hawaii
Student Health Policies
FROM THE HEALTH ROOM

Welcome to the 2004-2005 year at St. Joseph Junior Senior High School We are truly happy to have the opportunity to work with each and every one of your children.

Please read over the following items and feel free to call us if you have any questions -- (808) 935-4936.

1. Health requirements
All children must meet the State of Hawaii health requirements in order to attend St. Joseph School. No one is allowed in school without a valid Tuberculosis Clearance Certificate. This is a PPD or chest x-ray performed in the United States within one year prior to admission to school in Hawaii. All students also need a current physical examination and must be up to date on their immunizations. If you have any questions about the requirements, please call the school office and ask to speak to one of us.

2. Health Questionnaire   Online Questionnaire Form    Printable Questionnaire Form
We are attaching a Student Health Questionnaire so that we may be aware of any medical condition that your child may have. You may submit it online or you may print out a form and submit it in writing.

3. Emergency Packs
Last year our Crisis Management Team instituted a policy that each student must have an Emergency Pack available for their use in case of an emergency at school (hurricane, flooding, earthquake, etc.). Currently not all students have turned them in to the Health Room. We are asking that you have your child bring in his or her Emergency Pack as soon as possible.
    EMERGENCY PACK CONTENTS
  • 1 liter bottle of water
  • 1 small flashlight
  • batteries for flashlight (in package)
  • 2 kleenex tissue travel packs
  • 6 granola bars or cheese and cracker packs
  • emergency blanket (space blanket)*
*Made of mylar. Available at Hilo Surplus Store on Mamo Street for $3.98

Place everything except the water in a 1 gallon Ziploc bag. Write the student's name on the outside of the bag and on the watter bottle. Have your student bring it directly to the health room. At the end of each year the packets will be returned to the student to be updated and returned the following fall.

If you are unsure whether you turned yours in, just give us a call

4. Kleenex tissues
Each classroom, as well as the health room, goes through a lot of tissue, expecially during cold and flu season. The health room budget does not allow us to supply all of the students with tissue. We ask that each student bring 2 boxes of tissue to the health room at the beginning of the year, and we will keep the classrooms supplied. This will help us out tremendously. Thank you.

5. Medication administration
With the exception of asthma inhalers, students are not allowed to carry medication with them at school This is for their safety as well as the safety of other students. We are not allowed to give out any medication (including over the counter medication) without a written order from the child's doctor. If your child has any medical condition that requires administration ofmedication during school hours you must have a Medication Administration Form completed by the doctor and on file in the health room. Asthma inhalers may either be stored in the health room or carried in the student's backpack. Either way, a completed Medication Administration Form must be on file in the Health Room.

6. Athletic Physicals
All athletes are required to have a current athletic physical on file in the health room prior to practicing or participating in any sport sanctioned by the BIIF and sponsored by St. Joseph School. Athletic physicals need to be updated yearly. You may take your child to his/her regular physician. Once or twice each year we try to have a physician in to do the physicals at school. You may call us to see if a date iss cheduled. Forms are available in the Athletic Office, the Health Room and the School Office. Please make sure that your athlete has this completed prior to any athletic participation.

In closing, we want to remid you that we are available to evaluate students with minor illnes and injury. We will contact you if we feel your child needs medical treatment or further evaluation. We will store and dispense prescribed or over the counter medications with proper documentation. We work one on one with the students at every opportunity to promote wellness and encourage good health habits. If you have any questions or concerns, please do not hesitate to give us a call.
